Submit a Work Order

Work Orders may be submitted in person at our office (TAH 1023, adjacent to the faculty mailboxes), or as e-mail attachments. If you wish to e-mail a document for duplicating, attach the file to be duplicated, along with a completed Work Order Form to an e-mail and send it to: cbaiso@csus.edu.

A separate Information Services "Work Order Requisition" form must be submitted with each duplication, word processing or graphic design request, whether the request is submitted in person or via e-mail.

The Work Order Requisition is available in two versions, a PDF file and an Excel template. The PDF version has "interactive" fields that you can fill out if you have Adobe Acrobat Professional. Alternatively, if you have only the Reader version of Acrobat, you can print out the form and fill it out manually. The Excel template can be saved to your computer and used as needed - printed out or attached to an email.

PLEASE - ONE attachment and ONE work order only per e-mail.

We have been getting e-mails with multiple attachments, and long-winded messages with confusing instructions for each attachment. Senders are getting mixed up about what they have or have not attached, and the instructions don't always match what is attached. To prevent confusion --- ONE attachment and ONE work order per e-mail.

Please submit on-line orders to cbaiso@csus.edu instead of our individual e-mail addresses.

Restrictions

Do NOT submit books or magazines with pages marked for duplication. Pages from publications must be removed prior to duplication, or you may have copies of the pages made elsewhere and we will duplicate the copies.

We will prepare research materials ONLY for small projects and only if time permits, pending review and approval by ISO staff. No research projects will be done during the summer, as ISO has only one staff person during summer sessions.

Turn-around
times

If your work order includes typing in addition to duplication, please allow three (3) full working days for turn-around time. "Typing" includes creating exams from test banks. "Working days" do not include weekends!!

For "duplication only" work orders, we can usually handle rush orders, however, if you have a rush order, please discuss it with an ISO staff member. Don't just drop off a rush order with the assumption that we will be able to complete the job in the time requested.

Adobe Acrobat PDF version of Work Order Form

Excel Template version of Work Order Form

The Work Order form is REQUIRED for ALL jobs submitted to ISO, whether dropped off in person or e-mailed to us. Orders submitted on-line without the form will not be processed until the associated order form is received, either attached to an e-mail message or as a hard copy form dropped off at ISO.

NOTE: If you attach the order form to an e-mail, ISO staff will fill in the "Work Order Number" field. The Work Order form contains all required information for ISO to complete the order, so it is not necessary to write anything in the body of the e-mail message unless you wish.

The links at left are for two versions of the ISO Work Order Form. Adobe Acrobat is required to download the PDF form. If you do not have Adobe Acrobat Reader, you can download it free from the University Computing and Communications Services website at: http://www.csus.edu/uccs/software/ (click on the Obtain Software button). Acrobat Reader is also available for free on the Adobe website at:
http://www.adobe.com/support/downloads/main.html

Please note that the professional version of Adobe Acrobat (which is not available as a free download) is required if you wish to save a PDF document on your computer. The free version (Acrobat Reader) only allows you to open and print the blank Work Order form.

The PDF form can be used two ways:

1) If you have Acrobat Professional, you can save the file to your computer, and fill out the fields. To fill out the fields, click where you want to enter information and you will see a blinking cursor - start typing. Hitting the "Tab" key will take you to the next field. Click in the check boxes where appropriate. After you fill out the form you can either print it out and attach it to your hard copy originals, or save it on your computer and attach both the order form and your original document to an e-mail.

2) If you have Adobe Acrobat Reader (see the instructions above for downloading the free version), you can print out the PDF form , fill it out manually and attach it to the hard copy originals, then drop everything off at the ISO office.

If you are e-mailing an order to ISO, remember to attach the file to be duplicated AND the Work Order Form to the e-mail.
